Why headcount is the first question

Because it is the only one with a hard boundary in it. HMRC recognises 6 free products, and all of them are free only for employers with fewer than ten employees. Cross that line and the free tier closes, which is the single biggest change in what you will pay. Above fifty, most suppliers stop publishing prices at all and move you to a sales conversation.
